"Laser Hair Removal"



DESCRIPTION OF PROCEDURE

Laser hair removal has become one of the most common cosmetic procedures performed all over the world.
Laser hair removal utilizes beams of highly concentrated light designed to selectively penetrate into the hair follicles, to be absorbed by the pigment in the hair follicles and to destroy the hair within that hair follicle.

A variety of lasers and light sources have been developed that may be employed in laser hair removal.

Various types of lasers are utilized with laser hair removal – the most common of which are alexandrite, diode, Neodymium YAG and intense pulsed light sources.


WHAT IT CAN HELP/CURE

Laser hair removal can remove unwanted hair in those individuals who have disease states in which unwanted hair growth can occur, or it can be performed on those individuals who have no active disease states and just want removal of unwanted hair.

Most individuals will require a series of treatments over time which will result in the removal of the unwanted hair.

Most of the lasers and light sources are FDA cleared for permanent hair reduction, which means reduction over time. Some individuals will see very long term hair removal which may last for many years.

Others may require maintenance treatments over time to maintain their degree of hair loss. It is truly impossible to determine in advance who will require how many treatments and how long the hair will remain gone.



 

WHAT TO EXPECT BEFORE, DURING AND AFTER THE PROCEDURE


Prior to having laser hair removal, thoroughly check the credentials of the physician or technician performing the procedure.

Although laser hair removal is the second most common cosmetic procedure, many clinics promise results that are just not realistic.

Once you have the proper comfort level with the clinic where you are going to have the procedure performed, you will schedule an appointment.

Most patients will have the hair trimmed with a scissor to just a few millimeters above the skin surface.

During the treatment :

The laser and light source will be set with parameters which take into consideration your skin color, the hair color, the thickness of the hair, the location of the hair and the amount of energy needed for the treatment to be performed.

Depending on the laser or light source used, both the patient and the technician will wear appropriate eye protection, to prevent the laser light from penetrating into the eyes, which in itself can be a significant danger.

The area to be treated should be clean and dry.

Again, depending on the laser or light source, either a cold gel or a special cooling device will be used to protect the outer layers of the skin. This also helps the laser light penetrate further into the skin.

After the treatment

When the procedure is finished, it is common for the area treated to have some redness and some accentuation of the hair follicles themselves. This is “normal,” and should disappear over the next 12-24 hours, in most cases.

At the conclusion of the procedure, most will have ice applied to the area treated, and some may use some special aloe gels or anti-inflammatory creams, lotions, or water to relieve any discomfort to the area.

Most individuals will be scheduled for another treatment 4-6 weeks later and the procedure will be repeated until hair growth has been eliminated.

At each successive laser hair removal procedure, less hair should be found in the treatment area.


COMPLICATIONS THAT CAN OCCUR

As with any laser treatment, complications can occur with laser hair removal.

With laser hair removal, redness and swelling of the hair follicle, known medically as erythema and perifollicular edema, are “normal” events s een with most laser hair removal procedures.

On occasion, pain and discomfort will occur with laser hair removal – most of this can be easily treated.

Other adverse events,these include blisters and burns, which can occur with every one of the devices on the market, and especially if there is a recent history of sun exposure or if one goes into the sun immediately after the procedure.

Patients should be advised to avoid direct sun exposure for at least one week following all laser hair removal procedures.

All patients should be warned of these risks prior to laser hair removal procedures and all facilities should be well versed in how to handle these complications.

Dermasurgeons are well versed in handling any of these complications, making these highly trained physicians and their facilities ideal locations for laser hair removal procedures.


QUESTIONS TO ASK YOUR DERMASURGEON

As with any cosmetic procedure, ask your dermasurgeon questions regarding their experience and training, including the training of any of the technicians who may perform the procedure.

You should be able to get some degree of expectations from your dermasurgeon, as to how many treatments he or she may think you may require and learn about the follow-up and how you go about speaking with the physician's office if you have any questions. Dermasurgeons are skilled in this and many cosmetic laser treatments.

"Laser Resurfacing"


What is Laser skin resurfacing?

An important advance in skin rejuvenation, laser resurfacing can diminish the signs of aging, treat skin damage, and correct acne scars.

Using a wand-like laser hand piece, undesired skin cells and wrinkles are softened or removed. Also called laser peeling, laser resurfacing is relatively bloodless and offers the dermatologist more control in the depth of penetration of the skin's surface, allowing increased precision and safety in treating delicate areas, such as around the lips and eyes. This popular procedure is known by several other names, including lasabrasion, laser peel or laser vaporization.

Ablative Lasers

Ablative lasers remove the top layers of the skin (epidermis) and heat the layer under the epidermis (the papillary dermis). This causes collagen (a substance that gives skin structure) regeneration. As the wound caused by the laser heals, smoother and more even skin is seen. There are three types of ablative lasers used for resurfacing: carbon dioxide (CO2), erbium:YAG (Er:YAG), and the long-pulsed erbium:YAG

How does the carbon dioxide (CO2) laser work?

In contrast to the previous version of CO2 lasers, the newest generation of pulsed CO2 lasers delivers short bursts of extremely high-energy laser light. This revolutionary technology actually vaporizes the undesired skin tissue one layer at a time, revealing fresh skin underneath. Often referred to as a pulsed CO2 laser, the laser's highly focused beam enables the dermatologic surgeon to gently remove the skin's surface with a lower risk of scarring and other complications in properly selected patients. Healing generally occurs over a two-month period.

How does the erbium:YAG (Er:YAG) laser work?

The high-powered Er:YAG laser produces energy in a wavelength that gently penetrates the skin, is absorbed by water (a major component of tissue cells), and scatters the heat effects of the laser light. These unique properties allow dermatologists to remove thin layers of skin tissue with exquisite precision while minimizing damage to surrounding skin.

The Er:YAG laser is commonly used for skin resurfacing in patients who have superficial to moderate facial wrinkles, mild surface scars, or skin discoloration.

The gentle touch of the Er:YAG laser means it is uniquely effective in rejuvenating sun-damaged skin in areas such as the delicate skin around the eyes and mouth, where other lasers may cause scarring.

Other body areas, such as the neck and hands, also may be considered for treatment with the Er:YAG laser.

Skin rejuvenation with the Er:YAG laser may offer the advantages of reduced redness, decreased side effects, and rapid healing – about 5-7 days.

How does the long pulsed erbium:YAG laser work?

A new generation of long-pulsed laser delivers results that fit in between those of the CO2 and the Er:YAG. The long-pulsed erbium:YAG laser provides more wrinkle relief and offers a reduced risk of scarring compared to both. However, with less heat to the skin, results may not be as striking.

Non-Ablative Lasers

Non-ablative lasers have gained immense popularity in recent years because they require little to no healing time.

They work by bypassing the surface of the skin and treating the layers underneath, known as the papillary dermis. This allows the skin to respond as if it were repairing a wound and skin regenerates.

With this technique, the epidermis is generally not removed. Because they are less intense than ablative lasers, non-ablative procedures pose minimal downtime and recovery.

One popular type of non-ablative laser uses an infrared light source that penetrates through the dermis to generate collagen and cause skin contraction and tightening.

Non-ablative treatments require multiple sessions, and may take several weeks after the last session for the optimal results of treatment to be seen. How laser resurfacing is done

What conditions can laser resurfacing treat?

Laser resurfacing is performed in the dermatologist's office to help:

Improve fine lines and wrinkles of the face, especially on the upper lip, cheeks, and forehead
Smooth and tighten eyelid skin
Improve crow's feet around the eyes
Soften pucker marks and frown lines
Remove brown spots and splotchy skin color
Improve and flatten scars
Repair smoker's lines


What are the benefits of laser surgery for facial resurfacing?

Laser resurfacing may offer the following general benefits

Improved treatment results
Precisely controlled surgery
An additional or alternative to other skin rejuvenating procedures
Safe, reliable, and effective out-patient, same-day surgery

What can be expected during and after laser resurfacing?

Depending on the specific condition being treated and the type of laser used, patients may receive a topical anesthetic cream, sedation, local anesthetic and/or monitored intravenous sedation.

Discomfort is usually minor both during the procedure and throughout the recovery phase. The time required to complete a full-face ablative procedure is generally between one and two hours, although this may vary depending on the sites treated.

Following ablative resurfacing, the treated areas usually are kept moist with ointment or occlusive dressings (surgical bandages) for the first few days. The skin is typically red or pink and may be covered with a fine crust.

The treated sites must be protected from sunlight after the procedure.

Once healing is completed, sunscreen should be worn every day. Depending on the type of laser used and the individual's ability to heal, a pink surface color may remain for several days to several months. After seven to fourteen days, makeup can be worn.

Non-ablative resurfacing treatments may take only a few minutes. Mild redness may persist for a few hours with non-ablative techniques and makeup may be applied immediately. Four to six treatments are usually necessary because the results from non-ablative techniques are generally less than those seen after Er:YAG and CO2 laser procedures.

What are the possible complications of the procedure?

All surgical procedures carry some degree of risk. With the new Er:YAG and CO2 lasers, the risk of scarring is low. Common side effects may include crusting, mild swelling, and redness or brown discoloration at the treatment sites. Some patients may require bleaching creams to help regulate skin color following laser resurfacing. Pain, bleeding, swelling, and infection are minimized with newer surgical techniques and meticulous post-operative care. Pre-operative medication may be given to avoid cold sores. Redness of the face for up to a few weeks is not uncommon and occasionally, may persist for months. Side effects are greatly reduced with the milder non-ablative lasers.

What are the limitations of laser resurfacing?

Laser resurfacing is not a substitute for a facelift, nor can the procedure eliminate excessive skin or jowls. However, beneficial tightening of loose skin can occur from laser resurfacing, resulting in an improvement of certain folds and creases.

In many cases, laser resurfacing offers an alternative to traditional methods for skin rejuvenation, such as dermabrasion and deep chemical peels. It can also work well in conjunction with or as an additional treatment to other aesthetic procedures such as chemical peels, blepharoplasty (eyelid surgery), liposuction of the face and neck, dermal fillers, radiofrequency tissue tightening and Botox.

"Laser Photo Rejuvenation"



Overview

Laser Photo Rejuvenation is a new treatment to help the skin look firmer and smoother. The non-ablative laser treatment is designed for people who are too busy or do not wish to go through the recovery time of deeper laser resurfacing.

This treatment utilises two or more lasers that specially target vascular (superficial blood vessel) and pigmentary changes (brown discolouration or spots),wrinkles,scrs in our skin that develop with age and sun damage, to provide you healthy younger looking skin.

 

 

The Procedure

In Laser Photo Rejuvenation a combination of lasers are specifically chosen to suit the patient's skin type and problem areas to improve the overall texture and colour of the skin.

A fine laser beam specifically targets individual spots and blood vessels whilst also causing thermal (heat) energy to the skin, stimulating cells (fibroblasts) to produce new collagen and result in skin tightening.

The sensation of light can be uncomfortable but a cooling device provides comfort to the patient by cooling the skin and ice may be applied if required. If the doctor elects to use some form of anaesthesia, all options would be discussed with the patient. It is not uncommon for a local anaesthesia cream to be applied half an hour to one hour prior to treatment if the patients desires.

How many treatments are required and at what interval? Is follow up necessary?

The number of treatments required depends on several factors including skin tone and the extent of pigmented and vascular lesions and the degree of sun damage that exists. Typically a minimum of three treatments (range 3-5) are performed at monthly intervals for optimal results.

After your initial series of treatments, it is possible to require a follow-up treatment in two or more years. Patients with rosacea may require regular maintenance treatments; frequency depends on the severity of the condition. We will educate you in ways of caring for your skin to help diminish the need for future treatments.

After the procedure

Laser Photo Rejuvenation patients may experience some initial redness and swelling to the skin, but this usually quickly subsides. Occasionally scabbing and bruising may occur, and these may take up to 10 days to subside. The treatment lasts 45 minutes and patients are advised that multiple treatments (3-4 treatments at monthly intervals) may be necessary to achieve optimal results. As this treatment not only improves the skin's texture and colour, but also stimulates the regeneration of collagen cells, improvement is gradual.

Risks And Complications

Minor problems may be redness, swelling, bruising and scabbing. These are all temporary. Other problems such as Hypopigmentation (lightening of the skin) and hyperpigmentation (darkening of the skin) are possible risks and complications of this procedure. The patient must also agree to refrain from sun exposure or tanning booths for 2 weeks after treatment and failure to comply with post-treatment skin care instructions may increase the chance of complications.

Other Considerations

Laser resurfacing does not usually impose restrictions on lifestyle after treatment; however consideration should be given to social engagements and best planned a few weeks later than treatment.

What Lasers Are Used For Acne?

We will often use more than 1 laser or light based system to optimize the acne clearing benefits of these devices. How Many Treatments Are Required?

Most patients require approximately 5-7 monthly treatments to see the desired improvement.
Periodic re-treatments will be required as acne flares.

What Does Laser Acne Treatment Feel Like?

Treatments generally have the sensation of a rubber band. For those who find this uncomfortable, a numbing topical anesthetic cream can be used.

Can I Combine Laser Treatment With Other Acne Treatments?

Most patients stay on their topical medications while undergoing laser treatments. In general, less antibiotic treatment by mouth will be required. Often microdermabrasion when combined with acne laser treatment, leads to even better results.

"Laser For Growths On the Skin - Carbon Dioxide Laser Skin Resurfacing"

Carbon dioxide lasers have been in use for many years to treat different skin conditions. A new generation of carbon dioxide lasers use very short pulsed light energy (ultrapulsed) or continuous light beams that are delivered in a scanning pattern to remove thin layers of skin with minimal heat damage to the surrounding structures.

These lasers are used to treat wrinkles , scars, warts, moles, skin tags, freckles, linear epidermal nevi ( birthmarks ) , rhinophyma (enlarged oil glands on the nose), and other skin conditions.

Carbon dioxide laser resurfacing is usually performed on an outpatient basis, using local anesthesia in combination with orally- or intravenously-administered sedative medications. The areas to be treated are numbed with a local anesthetic. (General anesthesia may be used when the entire face is treated.) A partial-face laser abrasion takes 30-45 minutes, and the full-face treatment takes 1-1/2 to 2 hours.

Following the laser resurfacing procedure, a special dressing is applied to the treatment sites for 24 hours. The areas usually heal in 10-21 days, depending on the nature of the condition that was treated.

Side effects include:

Milia (small white bumps) in the laser-treated areas. These may be removed by gentle cleansing with a washcloth.

Hyperpigmentation , and sometimes, hypopigmentation

Reactivation of herpes simplex cold sore, especially after laser resurfacing around the mouth. This can be prevented by giving an antiviral medicine before the surgery and continuing it for seven to 10 days afterwards.

Bacterial infections. These can be prevented by taking an antibiotic prior to the surgery and continuing for seven to 10 days afterwards.
